Where do you get WIC?

WIC has many clinics in this area.   Call 740-594-8143 or 1-800-516-0339 in Athens County or 740-342-4938 or 1-800-709-6483 for Perry County.

What you need to bring when you apply:

You will need to bring specific information and documents to apply for WIC.

  • Identification for all applicants such as driver's license, birth certificate, baptismal certificate, Medicaid card,   immunization records, etc.

  • Proof of residency will be requested.  If so, bring a recent utility bill, business mail, driver's license, or rent/lease receipt.

  • Papers providing participation on AFDC, Food Stamps, or Medicaid that contain your name and your child's name.

Or, provide the name, and date of birth for everyone in the household who works (most recent paycheck stubs - for the past thirty days), income tax forms: W2 or 1040, or unemployment records.
